Our medical office is located in Metro Louisville. Dr. Omoruyi is an established & independent medical provider. He is board certified in family practice, primary, geriatrics and palliative care. The office staff at Pearl Medical is experienced with all types of insurance filings and self pay.

Office Policy for Co-Pays
Payments of all co-pays, deductibles and/or coinsurance are due at the time of service.
Office Policy for Controlled Substances
Pearl Medicals office policy for patients receiving a controlled substance is to utilize confirmatory urine drug testing to confirm compliance with prescribed medications as well as rule out illicit drug use.
This test may not be covered by your insurance company but is required by this office. We will be collecting $85.00 for all patients requiring a Urine Drug Screen whose plan does not cover this testing. This will be due at time of service and necessary for the provider to prescribe controlled substances. Please complete the form below and bring it with you to your visit.
